The information I have provided in this application is true and complete to the best of my knowledge. [ understand and agree that any omission or misstatement contained in this application or other material I may provide to Nelson Wood Shims (eg, a resume), shall be grounds for rejection of this application or, if I am hired by Nelson Wood Shims, immediate termination of my employment.

I understand and agree that if I am hired by Nelson Wood Shims, 1 will be employed on an at—will basis. That means that either Nelson Wood Shims or I may terminate the employment relationship at any time and for any reason, or for no reason. with or without prior notice. I understand and agree that nothing contained in this application, conveyed during any interview which I may be granted, or contained in any company policy statement, is intended to alter the at-will employment policy or to otherwise create an express or implied employment contract between me and Nelson Wood Shims I further understand and agree that nobody at Nelson Wood Shims, Inc, other than the Human Resources Manager, can alter the at—will policy or enter into an employment agreement with me for any specified period of time, and that even the Human Resources Manager can only enter into such an agreement if it is in writing and signed by the Human Resources Manager.

I authorize Nelson Wood Shims to contact and thoroughly investigate my references, work history and other information provided in this application and/or my resume in order to make an employment decision. Unless I specifically stated otherwise in writing on the application, I authorize my current and former employers, my schools, and my listed references to disclose to Nelson Wood Shims any requested information. I release Nelson Wood Shims and its employees, agents and representatives, my current and former employers and their employees, agents and representatives, my current and former schools and their employees, agents and representatives, and the listed references from any and all claims or liability arising out of or associated with such investigation or disclosure.

I am not subject to a non-competition, non-solicitation, confidentiality or non-disclosure, or any other agreement that would interfere with or prevent me from working for Nelson Wood Shims

If I receive a conditional offer of employment from Nelson Wood Shims, I understand that the offer is contingent upon the following:

- I will be required to consent, submit to, and pass a drug test pursuant to Nelson Wood Shims’s drug testing policy. I understand that I have the legal right to refuse to be tested, but that if I refuse the conditional offer of employment will be revoked and I will not be hired.

~ I will need to pass a criminal background check.

- I will be required to provide proof of my identity and that I have the legal right to work in the United States.

- I will be required to execute a Non-Compete Agreement.

If I am hired by Nelson Wood Shims, I agree that I will comply with any and all Company policies and procedures.
